- Treatment: Sharing information with doctors, nurses, therapists, and others involved in your care to provide the best treatment possible. This may include sharing information with family members or home care providers after discharge.
- Payment: Sharing information with your health plan or third parties to bill for services and obtain prior approvals.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with iSmart Home Care or with the Secretary of the Department of Health and Human Services. To file a complaint with us, contact Tywan Mata at TMata@ismarthomecare.com. Complaints must be in writing, and you will not be penalized for filing.
