Toniane is a seasoned medical and mental health services leader with 15 years of progressive management and leadership experience in service administration and contract management. Through her support and oversight of programs, Toniane has ensured and delivered access to quality programming for thousands of service recipients.
Roxanne is a dedicated and highly motivated nursing leader with a rich background spanning 9 years in the healthcare field and is now contributing her expertise to iSmart Home Care.
Born and raised in Clifton, NJ, Roxanne has not only built her career in her hometown but also raised her family there. Her healthcare journey commenced during high school when she pursued training and earned Emergency Medical Technician (EMT) certification. Roxanne's commitment to community care was evident as she worked and volunteered as an EMT in various communities, including Franklin, Orange, and Irvington, all while pursuing her education at Rutgers University. In 2010, she proudly graduated with a bachelor's degree in psychology.
Shalana Farrell serves as the dedicated administrative assistant at iSmart Home Care. Armed with a genuine passion for delivering outstanding care and support, she contributes a wealth of experience and specialized skills to the team.
Her primary role is to facilitate the smooth and efficient functioning of the organization. With a strong background in administrative support and a nuanced understanding of the home care sector, Shalana is adept at assisting both clients and the internal team in various capacities.
Tywan D. Mata is a dedicated and passionate healthcare professional with a deep-rooted commitment to community service and improving the lives of others. Born and raised in the vibrant neighborhood of East New York, Tywan has always felt a strong connection to the community and a drive to make a positive impact.
Tywan’s journey in the healthcare field began in 2007 when he joined CAMBA’s Women’s Prison Association as a Case Planner. Working with incarcerated individuals, Tywan witnessed the importance of compassion, empathy, and support in the rehabilitation process. These early experiences fueled his desire to pursue a career centered around helping others and fostering positive change.
In 2011, Tywan transitioned to CAMBA’s Health Home Program, where he embarked on a journey of growth and responsibility. Over the years, he took on progressively challenging roles, starting as a Program Supervisor, then moving up to Program Supervisor II, and finally assuming the position of Program Director. In these capacities, Tywan honed his leadership skills, managing daily operations and collaborating with matrixed teams to ensure quality healthcare delivery. He felt privileged to serve as a point of contact for various stakeholders and contribute to the improvement of operational efficiency and communication.
Tywan’s dedication to community welfare extended beyond his professional roles. Since 2000, he has proudly served on the Board of Directors at Housing Works Health Care Inc., actively participating in shaping the organization’s mission and advocating for the needs of vulnerable populations. It brings Tywan immense satisfaction to contribute to the delivery of high-quality healthcare services and make a lasting impact on the lives of those in need.
In 2015, Tywan joined the Community Healthcare Network (CHN) as the Regional Deputy Director, overseeing the daily operations of CMA within the CHN Lead Health Home Program. Here, he continued to apply his skills in streamlining processes and implementing solutions to benefit both patients and staff.
In 2019, Tywan took on a new challenge and joined AIRnyc as the Director of Care Coordination. His dedication to patient-focused healthcare and strategic leadership soon led to his promotion to Chief of Care Coordination in 2021. This role allowed him to play a critical part in ensuring the continuity of caregiving during the trying times of the COVID-19 pandemic. By optimizing resources and fostering a positive work culture, Tywan aimed to enhance efficiency and stakeholder satisfaction.
Tywan’s commitment to professional growth led him to pursue higher education. He earned a Master of Social Work from Adelphi University in 2019, a Master of Public Administration with a focus on Public Affairs and Administration from Metropolitan College of New York in 2012, and a Bachelor of Science in Legal Studies (with a Minor in Psychology) from John Jay College of Criminal Justice, The City University of New York in 2007.
Outside of his professional endeavors, Tywan finds great joy in engaging in community service and giving back to the neighborhood. His unwavering commitment to community welfare drives him to make a positive impact on the lives of others, making Tywan a true asset to East New York and the broader community he holds dear.
Born in Trinidad and Tobago, Marlon Stewart’s roots have undeniably contributed to his dynamic and adaptive leadership style in the retail sector. With over 16 years of transformative leadership under his belt, Marlon seamlessly blends strategic vision with adept business acumen. His deep commitment to team development is evident in his extensive experience in personnel management, spanning hiring, performance evaluations, promotions, and more.
Marlon’s entrepreneurial flair is most pronounced in his knack for cultivating pivotal relationships with vendors and business partners, a strategy that has propelled substantial revenue growth. As he looks ahead, Marlon is poised to harness his diverse skill set to elevate iSmart Home Care to unparalleled success.
Synonymous with contemporary retail leadership, Marlon represents not just expertise, but the evolution and future of the industry. His career is more than a testament to experience; it’s a legacy of continuous growth and innovation.
Phil Sharpe is an accomplished social worker with over 18 years of combined experience in client care and project management. Having specialized in supporting those with HIV/AIDS and varied medical and mental health challenges, his proficiency is evident. Holding both a Master’s and Bachelor’s degree in Social Work from Lehman College, Phil is academically and professionally equipped to deliver exceptional care.
In his current role, he adeptly assists residents in securing entitlements and collaborates seamlessly with service providers for comprehensive care. Earlier in his career, as a Program Supervisor, Phil demonstrated commendable leadership, overseeing a team of Care Coordinators and pioneering efficient auditing processes. Additionally, Phil’s dedication to the HIV/AIDS community is underscored by his efforts in helping patients navigate essential services and government benefit applications.
Throughout his tenure, Phil has consistently balanced compassion with efficiency, leaving a lasting impact on the individuals and communities he serves.
Toniane was born in Jamaica, West Indies where she spent her childhood years then migrated to Brooklyn, New York where her passion for the community and people was developed.
She has transformed Programs through efficient hiring practices which led to growing programs to multiple locations in New York. Some of her accomplishments include: slashing staff turnover rate by 30% in less than a year and CAMBA’s Health Homes Program awarded advance funding due to efficient hiring success, improved staff morale and productivity through creation of streamlined operational processes and successfully led the lowest performing location (1 of 3) into being the highest performing within one year.
Toniane has managed budgets, fund allocation, procurement, payment, risk, and compliance for a portfolio of contracts with a diverse set of revenue sources (including New York City Tax Levy, New York State A and 6, Federal, and private funds). Often collaborating with subject matter experts in Finance and Legal Affairs.
Toniane received a Bachelor of Arts Degree in English at Stony Brook University where she also studied Pre-Med. She then went on to receive a Certificate in Culinary Arts, then obtained a Masters in Public Administration with Membership in Pi Alpha Alpha National Honor Society for Public Administration.
Toniane loves to bask in nature as well as learning and teaching others about holistic living. She’s a proud “tree hugger” who enjoys spending time with family and loved ones.

Since 2018, Roxanne has primarily focused on the field of home care, where she has not only excelled but also taken on roles in supervising and educating Certified Home Health Aides (CHHAs) to ensure the highest quality of care.
In 2020, Roxanne’s dedication and leadership were recognized when she was promoted to the position of Director of Nursing (DON) at her previous home care employer. During her tenure as DON, Roxanne orchestrated the formation of a cohesive nursing team, implemented vital policies, and revamped clinical documentation to align with industry standards and accreditations. Her work ethic is founded on honesty, patient advocacy, and a fair and equitable work environment promotion.
Outside of her professional life, Roxanne enjoys spending time with her husband and three children. Her commitment to helping others extends beyond her work hours. She has actively volunteered with local charities like Power of One, providing essential assistance, including food and clothing, to Clifton residents in the aftermath of Hurricane Ida. Roxanne’s dedication to her community is further evidenced by her involvement as a volunteer in the school system and her contributions as a class parent and participant in t-ball events, where she imparts positive values to shape the lives of future generations.
